This post will give detailed information about the University College Hospital (UCH) Ibadan School of Health Information Management (SHIM) HND Admission Form for the 2024/2025 Academic Session. More official details about the form are given below;
Applications are invited for admission into the School of Health Information Management of the University College Hospital, Ibadan, for a Two-Year Higher National Diploma (HND) Programme in Health Information Management. The non-residential course will commence in September 2023.
Entry Qualification for the HND Programme
Applicant must present his/her ORIGINAL CERTIFICATES.
(a) O’ level Result
(b) National Diploma or Professional Diploma in Health Information Management
(c) License Certificate
(d) Letter of completion of Industrial Training (IT)
Entrance Examination
The Computer Base Test (CBT) for ND will be in:
(a) English Language
(b) Mathematics
(c) Physics
(d) Chemistry
(e) Biology
The Computer Base Test (CBT) for HND will be in:
(a) Health Records Management
(b) Anatomy & Physiology
(c) Clinical Coding
(d) Biostatistics
(e) Computer Science
(f) Health Planning Management
How to Apply
The application form is available from Monday, 27th May 2024, through the UCH SCHOOLS PORTAL: https://portal.uch-ibadan.org.ng. Interested individuals are to do the following:
1. Visit the UCH schools’ portal at https://portal.uch-ibadan.org.ng and click the REMITA PAYMENT button to make a payment.
2. After successful payment, candidates are to sign up with his/her RRR. Email and Password used for the payment.
3. Login with your Email and Password to continue the online application
4. Go to the portal on https://portal.uch-ibadan.org.ng and download the Applicants User Guide.
5. Follow the instructions in the Applicants User Guide to input your data and submit your application online.
6. Print two copies of your form and photo card and bring them to the examination centre on the day of the examination.
7. Make a payment of another Five Hundred Naira (N500.00) only into Health Information Managers Association of Nigeria, Account no: 0000207307 Union Bank, UCH Branch
The screening exercise for the ND programme and HND entrance examination will be communicated to the candidates via their e-mail addresses. Successful applicants will be invited for an oral interview via text messages and the School’s website.
